Answer 1:
This is most likely an unintended summary bar. What is likely happening is that a task has been indented (outlined). When a task is indented and summary bars are turned on for the schedule, symbols added to lower level bars may appear on their summary bar which is always the first higher level task above it. Diagnose this issue by adding an Outline Level SmartColumn to the schedule. If the schedule is intended to have all tasks at the same level, just outdent the tasks until all levels are 1. If summary bars are not needed, it's also possible to just turn off the summary bars. (Layout | Summary Bar Settings...)
Answer 2:You might have some hidden symbols on the schedule.
